We provide specialist case management services for children, adolescents, and adults with complex physical disabilities. With over 20 years of clinical experience, our approach is grounded in person-centred, goal-oriented planning that supports functional outcomes across all life stages. Services are delivered in homes, schools, and community settings throughout NSW, under NDIS, iCare (Lifetime Care and Support), and private funding arrangements.

Case Management Services

We offer case management for children and adults with spinal cord injury across NSW under the Lifetime Care and Support Scheme, as well as other funding bodies.

This includes:

• Coordination of multidisciplinary teams

• Ongoing goal monitoring and adjustment

• Functional progress tracking

• Liaison with medical specialists, therapists, schools, and service providers

• Advocacy to ensure funding aligns with evolving needs

• Support with life-stage transitions (e.g. school to adult services)

• Guidance through complex systems like iCare and NDIS
